   <subfield code="a">Summary: Background: Wedge resection for undefined pulmonary nodules can not always be performed with only minimally invasive technique. Preoperative knowledge about the exclusive videoendoscopic resectability would save operating time. Methods: We evaluated 57 Patients with indeterminate pulmonary nodules to determine whether our previously reported theses about the preoperative features indicating videoendoscopic resectability is still tenable. From March 1992 to January 1995, 57 patients underwent videoassisted thoracic surgery (VATS) for undefined pulmonary lesions. This group consisted of 32 men and 25 women their age ranging from 25 to 77 years. We stratified this group of patients according to the location of the nodules peripheral or central in either lobes. Differences in the conversion rate (technical impracticability to remote the lesion with an atypical resection by VATS, subsequent thoracotomy) were tested by Fisher's exact test looking for statistic significance. Results: This analysis revealed highly significant differences in the conversion rate between the group of central nodules in the upper lobes and all the other groups together. Conclusions: These findings strongly support our recently published theses with statistical significant data and confirm our operative strategy i.e. excluding centrally located nodules in the upper lobes from the videoendoscopic approach.</subfield>
